One-time enrollment fee of $92 (waived for patients with history of a dental savings plan)

This is not dental insurance; it is a dental discount plan that cannot be combined with any other dental insurance.

The plan is good for Graceful Dentistry only. Therefore, if you are referred to a specialist, they will not offer this discount plan.

Payments are due at the time of service. Should you decide to extend payment for treatment by paying through CareCredit, the discount is reduced to account for merchant fees.

This discount plan cannot be used should there be dental treatment needed following any type of injury where a lawsuit, disability, or workman’s compensation type insurance is involved.

This plan is non-transferable : Plan is specific to each person. Family members cannot be substituted for other family members.

This plan is non-refundable : A patient who chooses not to use their discount dental plan will not be issued a refund.

Discounts do not include products sold in our office (i.e. fluoride toothpaste, Oral-B toothbrush, teeth whitening systems or gels ).

Exclusions and limitations may apply – ask at the front desk if something is covered under your dental plan if not listed under ‘Discounted Dental Services’.

You will not be excluded from annual rate increases or cancellation fees. You will not receive a membership card.

Your plan’s effective date will be on file with our office and expires exactly one year from the date of purchase.
